How to organise an elderly parent's medical information
Each new doctor, every ER visit and most forms ask for the same things. Gathering them once, and keeping them current, saves hours and avoids mistakes made under pressure.
Gather these once
- Conditions and allergies, in the words the doctors used.
- The current medication list, copied from the labels.
- Every doctor's name, specialty and phone number, and the pharmacy.
- Insurance and Medicare cards, front and back.
- The healthcare directive and power of attorney, and where the originals are.
Keep it current
- Update the medication list the day anything changes.
- After each appointment, write down what was said and any new instructions.
- Check expiry dates on cards and documents a month ahead.
Have one page ready
